Metformin hydrochloride is an antihyperglycemic agent which improves glucose tolerance in patients with type 2 diabetes lowering both basal and postprandial plasma glucose. Metformin decreases hepatic glucose production, decreases intestinal absorption of glucose and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.
INDICATION AND DOSAGE:
Metformin hydrochloride sustained release tablets are indicated as an adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes. Metformin sustained release tablets is indicated in patients 17 years of age and older. It may be used concomitantly with a sulfonylurea or insulin to improve glycemic control in adults.
There is no fixed dosage regimen for the management of hyperglycemia in patients with type 2 diabetes. Dosage must be individualized. The maximum recommended daily dose of sustained release metformin in adults is 2000 mg. The usual starting dose of metformin sustained release tablets is 500 mg once daily with evening meal. Dosage increases should be made in increments of 500 mg weekly. If glycemic control is not achieved with 2000 mg once daily then 1000 mg twice daily should be considered. Patients receiving conventional metformin may be safely switched to sustained release metformin at the same total daily dose up to 2000 mg once daily.
