<h2 id="Introduction">Introduction</h2><ul><li>Prevalence of Metabolic Dysfunction-associated Steatotic Liver Disease (MASLD) is rapidly rising due to increasing rates of obesity and metabolic disorders steadily increasing across several regions of the world including Europe, the Middle East, and the America.</li><li>Currently, MASLD affects an estimated 38% of adults and 7-14% of adolescents, with projections suggesting this may exceed 55% of adults by 2040.</li><li>Dysbiosis-characterized by reduced microbial diversity, elevated Firmicutes/Bacteroidetes ratio, and increased Proteobacteria-promotes inflammation, intestinal permeability ("leaky gut"), and systemic endotoxin translocation, contributing to liver injury. Western diets worsen this by altering gut microbiota, enhancing hepatic fat and cholesterol accumulation.</li><li>Understanding the gut-liver-diet axis is key to advancing prevention and treatment strategies beyond pharmacology.</li><li>This review presents significant evidence that the gut microbiota plays a crucial role in mediating the beneficial or detrimental effects of dietary habits on steatotic liver disease.</li></ul><h2 id="Harmful">Harmful Dietary Patterns for MASLD</h2><ul><li>Unhealthy dietary habits adversely affect the composition of gut microbiota, playing a role in the onset and advancement of MASLD.</li><li>Diets that are high in calories, particularly those abundant in saturated fats, trans fats, cholesterol, and fructose-sweetened drinks, in conjunction with diets that are low in protein, facilitate the accumulation of lipids in the liver via microbiota-mediated processes. These alterations are significant contributors to the initiation of inflammation and the progression of fibrosis. These effects mitigate fatty liver accumulation and contribute to beneficial MASLD outcomes.</li><li>Other vital components of managing steatotic liver disease include physical activity (like aerobic exercise and resistance training), weight management (including weight loss and maintenance), and behavioral adjustments (such as stress management and optimizing sleep).</li><li>Therefore, integrating microbiota-targeted dietary interventions with these lifestyle changes is essential for achieving the best outcomes for MASLD patients.</li></ul>
