Impact of metabolic syndrome in LUTS due to BPH

<h2 id="objective">Objective</h2><ul><li>Benign prostatic hyperplasia (BPH) is a common age-related condition with multifactorial etiology.</li><li>This study aimed to evaluate the impact of co-existing metabolic syndrome (NtS) on treatment outcomes with combination medical therapy in patients presenting with lower urinary tract symptoms(LUTS) due to BPH.</li></ul><h2 id="methods">Methods</h2><ul><li>Seventy patients with LUTS-BPH, with and without NtS, were prospectively enrolled between September 2022 and January 2024.</li><li>Following informed consent, all patients received combination therapy with tamsulosin and dutasteride for two months.</li><li>Treatment outcomes were assessed using the International Prostate Symptom Score(IPSS), mean flow rate (HFR), and peak urinary flow rate (Omax).</li><li>Data analysis was performed using SPSS version 23.</li></ul><h2 id="results">Results</h2><ul><li>The reduction in IPSS was significantly greater in patients without HtS compared to those with MtS (p&lt; 0.001) (Table 1).</li></ul><figure class="image"><img style="aspect-ratio:1117/342;" src="https://cdn.megawecare.com/GHBY/Internal-Images/Comparision%20of%20change%20in%20internation%20prostate%20symptom%20score.webp" alt="Comparision of change in international prostate symptom score" width="1117" height="342"></figure><ul><li>Improvement in MFR was significantly higher in the non-MtS group (p &lt; 0.001) (Table 2).</li></ul><figure class="image"><img style="aspect-ratio:1105/339;" src="https://cdn.megawecare.com/GHBY/Internal-Images/Comparision%20of%20change%20in%20mean%20flow%20rate.webp" alt="Comparision of change in mean flow rate" width="1105" height="339"></figure><ul><li>Both groups demonstrated significant and similar improvement in Omax(p= 0.829).</li><li>Overall, patients without HtS experienced better symptomatic relief than those with HtS.</li></ul><h2 id="conclusion" data-section="conclusion">Conclusion</h2><ul><li>This study demonstrates thot metabolic syndrome (MtS) adversely influences the outcomes of combination medical therapy in men with BPH-related LUTS, as reflected by lower improvements in IPSS and mean flow rote (MFR) compared to controls.</li><li>These results highlight the need for clinicians to counsel patients with LUTS-BPH and co- existing MtS regarding adversely affecting treatment outcomes.</li></ul>
