Personalized Antidiabetic Pharmacotherapy in Geriatric Type 2 Diabetes

<h2 id="introduction">Introduction</h2><p>Prevalence of type 2 diabetes mellitus (T2DM} in older adults is rising globally, presenting substantial therapeutic challenges in medication selection for optimal efficacy-safety balance in this population.</p><h2 id="clinical-challenges">Clinical Challenges</h2><ul><li>Two critical clinical challenges are: (1) insufficient treatment of healthy older patients, and (2) excessive treatment of frail/ill individuals.</li><li>Expanded pharmacological options enable tailored antidiabetic selection aligned with individual clinical phenotypes and comorbidity profiles.</li></ul><h2 id="key-highlights">Key Highlights</h2><ul><li>Review examined personalized management via three sequential steps: (1) frailty-based patient stratification; (2) individualized glycemic targets accounting for hypoglycemia risk; (Table 1) (3) pharmacological selection balancing hypoglycemia avoidance with cardiorenal protection (Table 2).</li><li>Sulfonylureas and insulin monotherapy are de-emphasized; metformin-based combination therapy is preferred using DPP-4 inhibitors (enhanced safety), or GLP-1 receptor agonists/SGLT2 inhibitors for cardio-renal outcomes.
