Proton Pump Inhibitor in Patients with Atrial Fibrillation

<h2 id="aim">Aim</h2><p>To assess the potential advantages of proton pump inhibitors (PPI) co-therapy for upper<br>gastrointestinal bleeding (UGIB), whether for primary or secondary prevention, in patients with Atrial fibrillation(AF) receiving Direct Oral Anticoagulant (DOACs) from a nationwide population-based cohort.</p><h2 id="methods">Methods</h2><p style="text-align:center;"><img src="https://cdn.megawecare.com/GHBY/Internal-Images/ugib-params10629.png" width="876" height="500"></p><h2 id="results">Results</h2><ul><li>PPI group was associated with lower risks of hospitalization for UGIB and UGIB requiring red blood cell transfusion than non-PPI group (Figure IA).&nbsp;</li><li>PPI co-therapy was associated with a significantly lower risk of hospitalization for UGIB (Figure 1B).</li></ul><p style="text-align:center;"><img src="https://cdn.megawecare.com/GHBY/Internal-Images/rbc-transfusion-110441.png" width="876" height="500"></p><ul><li>Benefits of PPI on the risk of hospitalization for UGIB were greater in those with older age (275 years), higher HAS-BLED score (23), prior GIB history, and concomitant use of antiplatelet agent. (Figure 2)</li><li>Low-dose PPI was consistently associated with a lower risk of significant UGIB by 43.6-49.3% (P &lt; .001).</li></ul><p style="text-align:center;"><img src="https://cdn.megawecare.com/GHBY/Internal-Images/subgroup-analysis-110355.png" width="876" height="500"></p><h2 id="conclusion">Conclusion</h2><p>In this large cohort of patients with AF on DOAC, addition of PPI co-therapy has proven to be beneficial for lowering the risk of hospitalization for UGIB, particularly in high-risk patients.</p>
